Dear Central Family:
Earlier this week, we announced a COVID vaccination clinic that will be held tomorrow, (Saturday March 13) at the Central Activities Center. Medical Arts
Pharmacy will be hosting the vaccination clinic and they have agreed to set aside an hour from 12:00 noon to 1:00 p.m. for eligible Central members to come by the Activities Center to receive a vaccination. There are still about 25 slots available for vaccine appointments during the noon hour. If you are eligible for the vaccination, we want to encourage you to help us fill up these remaining slots. The vaccine being utilized is from Pfizer. It requires two shots, and the second shot vaccine clinic will be held on Saturday, April 3 at Central’s Activities Center.
To qualify for a vaccine appointment, you must be included in Phase 1A or 1B as defined by the Arkansas Department of Health. Please click
here to see if you meet the current criteria. There were many new categories added to the eligibility list for vaccines on March 8.
If you qualify for a vaccine, here is what you need to do:

- Please click here to print and fill out a a consent form from the Medical Arts Pharmacy website prior to your appointment. If you do not have
access to a printer, you can fill out a form onsite when you arrive for your appointment.

- Be sure to bring a picture ID with you to your appointment and any other documentation you may need to demonstrate your eligibility for the Phase 1A or 1B categories such as pay stubs, name badges, etc.
Like you, we are eager to do all we can to eliminate the impact of the pandemic. Receiving a vaccine is one of the key steps you can take. We hope that 108 of you take advantage of this opportunity. We also hope to host other opportunities as we move
